The U.S. employer submits the petition (I-360) to the appropriate Service Center of U.S. Citizenship and Immigration Services (USCIS) in the U.S. In addition to the fees, extensive additional documentation has to be added to the application, especially evidence that the employee meets the respective requirements.
IMPORTANT: For the EB-4 visa, Premium Processing is currently not available.

Please also note the information in the Visa Bulletin stating current availability of immigration visas and thus also about respective waiting times. In the EB-4 category, visas are usually always available.
After approval by USCIS, the applicant will be issued the green card in the so-called Consular Processing.
Furthermore, the applicant has the option of applying for "adjustment of status" in the U.S. (e.g. if he or she already has a valid non-immigrant status).
